Common Assessment Plan and Follow-up Template Maintenance
Use Assessment, Plan and Follow-up Template Maintenance to maintain templates consisting of up to 30 of the most common counseling, plan and follow-up items that a provider uses to record a patient encounter. An assessment plan and follow-up template allows you to quickly enter patient assessments and plans information during the Patient Eye Health Assessment/Plan Window. You can maintain a single assessment, plan and follow-up template or as many assessment, plan and follow-up templates as desired including templates that can be used for condition specific situations such as, diabetes or glaucoma.
Notes:
NDS recommends that your practice establish a standard list for the most common assessment plan and follow-ups. You should assign STD as the Template ID and assign Standard as the template name.

Opening Assessment, Plan, and Follow-up Template Maintenance, click More.
1. Click Setup from the Desktop.
2. Click Common Plan/Follow-up from the Setup menu.
Common Plan/Follow-up Window Example:
Creating a New Assessment, Plan and Follow-up Template, click More.
To create a new record, click New from the Toolbar.
Tip: To indicate that you are building a new template, the window clears, the cursor stops at Template ID for your entry.

Enter the unique identifier for the assessment, plan and follow-up template. |
Save |
All entries are automatically saved without further actions. |

To view all of the existing assessment plans, click List. From the list you can select the record to change or delete. |
Template Name |
Enter the name of the common assessment plan and follow-up template. |
Note: Although you will tab from field to field across each line, the information on each line is not necessarily associated. | |

Enter the plan item for the template. Note: Each line item in the list must end with a comma and a space (, ). |

Enter the visit type as the action/event for the template. |

To return to the Setup menu, click Back To Setup. |

To return to the Providers menu, click Back to Providers. |
Editing an Existing Assessment, Plan and Follow-up Template, click More.
CAUTION: All
entries are automatically saved without further entries.
1. To
display the record to be changed, enter the Template
ID and press [TAB].
Tip: If
you do not know the Template ID, you can click List to see a list of templates.
2. To position the pointer to the information that you want to change, press [TAB] or click the field using the mouse.
3. Enter the data changes.
4. To complete the edit process, click in any white space in the window.
Deleting an Existing Assessment, Plan and Follow-up Template, click More.
1. To
display the record to be deleted, enter the Template
ID and press [TAB].
Tip: If
you do not know the Template ID, you can click List to see a list of templates.
2. Click Delete from the Toolbar.
3. At the Delete Records prompt, click Delete to delete the record, or click Cancel to escape from the delete routine.
