Forms Printing Menu

The Forms Printing menu contains commands used to print the various types of claim forms for billing provider services.

Directions:

To open the Forms Printing menu, click the following sequence:
Billing Functions from the Menu Bar; and Forms Printing.
Keyboard shortcut: [ALT] [b] [f]

Billing Functions Forms Printing Menu Items
1500 Forms	Prints the universal claim form required by HCFA for submitting all Medicare claims. Private insurers may also require that claims be submitted using this claim form as well.

BMS Forms	Prints the BMS forms for Ambulance billing. Note: WebHelp for this topic is not available.
PAC Forms	Prints the PAC forms for Ambulance billing. Note: WebHelp for this topic is not available.
Mass Medicaid Forms	Prints the Massachusetts Medicaid claim forms.
UB-92 Forms	Prints the medical insurance claim forms used to submit insurance claims for a facility such as, a hospital or drug rehab facility (institutional claim forms). Note: Use this program to print UB04 claim forms.
