Field Description - Patient Zip Code

The Patient Zip Code is the postal code associated with this mailing address for this patient or address record.  If you enter a City Code as the patient City Name, the Zip Code fills in, and you can edit it.

Tip:  If you don't know the Zip Code, you can look it up on the Internet.  Refer to the United States Postal Service website, http://www.usps.com/zip4/.

• Patient Entry - Name & Address Area in the Demographics Window

• Patient Wizard - Demographics Area

• Patient Entry - Patient Address Maintenance

• Patient Entry - Insurance Policy Holder Area

Format:  up to 10 characters.  Recommendations:

• Enter the five-digit Zip Code.

• Or, enter the nine-digit Zip+4 Code with or without the dash (—).
