Field Description - Patient Guarantor First Name, Initial

The Patient Guarantor First Name and Initial is the first name and initial of the guarantor of the patient's account.  The Patient Guarantor First Name and Initial combined with the Patient Guarantor Last Name makes up the guarantor's full name.

Notes:  

• Data Entry notes:

• Enter the first name, a space, then the middle initial.

• Do not add a comma between the first name and middle initial.

• The guarantor must first be set up as a patient, even if they are not a patient and are only paying the bills.

• The guarantor's full name and address print on patient statements.
