Field Description - Employer/Lawyer City

The Employer/Lawyer City is the city or town name associated with the mailing address for the employer or law practice.  You can enter a City Code in the Employer/Lawyer City field to quickly enter the City, State and Zip Code information.

Note:  The Employer/Lawyer City information may print in box 9c of HCFA claim forms depending on who is financially responsible for the account.
