The UPIN is the Unique Provider Identification Number assigned to a provider. This number is assigned by the insurance carrier. You can enter the provider's UPIN for each Form Type that you have set up. The UPIN prints on the appropriate claim form.

You can view and maintain the Referral Form Type UPIN during Referral Maintenance.
Because you can tailor the labels of the nine Form Type fields, the field names in your software may not match the field names listed in the Web Help.
Tip: Frequently, other insurance carriers will use the same provider UPINs as Medicare assigns to a provider, though not always.
